Anatomy of the ankle

The causes of ankle pain are many and varied, so perhaps it would be a good idea to look at how the ankle itself is composed. Your ankle is the joint which is formed at the conjunction where your leg meets your foot and is known as the talocrural joint. It is known as a synovial hinge, meaning that it connects the ends of the fibula and tibia in your lower limb with the end of the talus bone in your foot. In other words, the significant weight of your body is distributed to your ankle, making it an essential part of proper balance and locomotion.

Your ankle is composed of two joints, the subtalar joint and the ankle joint. Your ankle joint by itself includes two smaller bones (the fibula and tibia) which, together, form a joint which enables your foot to bend itself upwards and downwards. Two further bones of the foot (the calcaneus and talus) then connect, to make the subtalar joint.

Next, come the ligaments and tendons. The most important tendon that you need for your mobility is the large Achilles tendon. This tendon is often one of the leading causes of ankle pain. Also, there are a dozen other ligaments which crisscross the ankle. When these muscles sag or fail, the result is commonly described as flat feet or fallen arches, and can itself be one of the main causes of ankle pain.

What causes ankle pain?

Of all the possible sprains that your musculoskeletal system can endure, ankle sprains are among the most common and are due to injuries to the ligaments of the ankle – typically tears that occur with sudden stretching motions to the inner or outer ankle.  Such is the case when you twist your ankle accidentally when playing basketball or other sports, or when you step off an uneven surface unexpectedly.            

The level of ankle pain you experience will likely depend on just how much of a sprain or ligament tear your injury has produced. In very severe cases of an ankle sprain, one will often not be able to walk or put any pressure on your affected foot, due to the instability your injured ankle is experiencing.

Why does my ankle get injured?

Ankle sprains are a common variety of ankle pain-causing injuries, which are especially prone to occur in individuals engaged in athletic activities (contact sports or running events) but can occur just as often to people who do not participate in these activities. An ankle sprain is an injury to the ankle which occurs when ligaments (tough, fibrous tissues which serve as connecting agents in the lower leg, foot, and ankle) are overly stretched, bruised, or partially or completely torn.

  • Repetitive motion
  • Stress and muscle tension

 

How do I know my ankle is injured?

In general, ankle sprains are graded as mild, moderate, and severe. In cases of mild sprain, the ligament will be overly stretched, causing a slight case of overstrain. Mild strains are characterized as being due to a strained and partially or severely torn ligament. A severe case of ligament sprain involves complete tearing, to the extent that your ligaments can no longer support or control your ankle joint. Ankle pain in these instances is usually severe initially and may be accompanied by a sensation of the ankle popping, as well as by immediate swelling from injured blood vessels that may leak fluid locally into tissues of the ankle. 

Common Symptoms

  • Ankle pain reaction at the site where the tear occurred
  • Reddening
  • Bruise
  • Swelling

 

What can I do to help relieve ankle pain?

The degree to which the ligaments of the ankle are injured is best evaluated by x-ray tests which can also reveal any fracture to the bones of the ankle as well.  With minor ankle sprains, the only treatment needed is the application of ice, rest, and limitation on putting weight on the ankle until it has healed sufficiently. Acute ankle sprains may thus require the use of crutches during recuperation to avoid further damaging the ligaments and elevation of the leg to reduce swelling.  Anti-inflammatory pain medications are also often recommended for minor sprains to relieve ankle pain and promote healing by reducing inflammation.

For patients with severe injuries and fractures to the ankle, immobilizing casts may be needed and may be required until the injury has healed sufficiently. In worst case scenarios, surgical repair may be required followed by physical therapy.
